The Filaria Antigen Test is a rapid blood test that checks for circulating filarial antigens (CFA), proteins shed by the adult Wuchereria bancrofti parasite. This parasite is the main cause of Lymphatic Filariasis, a mosquito-borne tropical disease more commonly known as elephantiasis, where long-term infection can lead to severe swelling in the limbs and other body parts.
What makes this test genuinely useful is timing. The older method for diagnosing filariasis relies on a microfilariae smear, which only works if blood is drawn at night, since the parasite's larvae circulate in the bloodstream on a nocturnal cycle. That's not exactly practical for most people. The antigen test gets around this limitation entirely. Because it detects antigens from the adult worm rather than the larvae, it can be done at any time of day and still pick up an active infection reliably, which is why it's become the preferred diagnostic approach.
Doctors typically order a Filaria Antigen test when:
- Swelling in the limbs, breasts, or genitals is present without a clear cause, especially in areas where filariasis is known to occur.
- A patient has lived in or traveled to a filariasis-endemic region, since exposure risk is a key factor doctors consider.

- Follow-up after treatment is needed, to check whether antigen levels have dropped in response to anti-filarial medication.
- Screening is recommended as part of public health surveillance in regions where lymphatic filariasis is still being actively controlled.
